Complete MIT’s COVID-19 Vaccine Eligibility Form

To the members of the MIT community,

Last Friday, President Reif wrote to our community to share the news that MIT has been approved by the Commonwealth of Massachusetts as an employer-based vaccination site for COVID-19, meaning that in the coming months MIT may vaccinate up to 50,000 students, employees, affiliates, and dependents following the phases that the state has established.

Today we are writing to ask you to help MIT Medical get ready to manage this enormous responsibility by registering yourself and your dependents. More information on the registration can be found below.

First, we want to be clear that you may have other, faster choices for getting vaccinated, and that you are under no obligation to get the vaccine at MIT.

Second, assuming you’d like to be vaccinated at MIT, we want to be clear that we have a distance to go before that will happen for most of us.

There are two factors that will influence when you might be able to receive the vaccine at MIT. The first factor is the statewide vaccination “phase” that we are in and who is eligible to receive the vaccine in that phase. (The registration we are asking you to complete will enable us to determine what phase you and your dependents fit within.) The second factor is how regularly and in what volume MIT receives supplies of vaccine from the Commonwealth.

To lay the groundwork so we can move smoothly and quickly within the state guidelines, we are urging you to complete MIT’s vaccine registration form. You may fill out this secure form by visiting covidvaccine.mit.edu.

This form allows you to:

  • Indicate whether you may want to receive the COVID vaccine from MIT
  • Provide basic demographic information needed to match you to the state phases
  • Input basic information for any family members and dependents, living in the same household, who may also want to receive the COVID vaccine from MIT

Whether or not you are interested in being vaccinated by MIT, it is essential that you fill out this form right away. Our ability to plan for our on-campus vaccinations, and to request appropriate quantities of vaccine from the Commonwealth of Massachusetts, depends on you!

To complete this form, you must be a current MIT employee, a current student, or approved to access campus through Covid Pass. Current patients of MIT Medical who do not fall into one of these categories will be contacted directly when they are eligible for the vaccine.

If you indicate that you are not interested in receiving the vaccine from MIT, we encourage you to seek vaccination opportunities elsewhere.

Your responses to this vaccine eligibility form will be kept private — not shared beyond those who need it for operational purposes — and will help MIT Medical most efficiently allocate its limited doses of the vaccine, which must be used within hours of a vial being opened.

Your responses are also non-binding. If you receive the vaccine elsewhere, or change your mind about receiving the vaccine, you can update your responses any time to let us know — and we hope that you will.
